The low-intensity extracorporeal shockwave
therapy in the treatment of chronic pelvic pain syndrome: 1-year follow-up

Purpose
Low intensity extracorporeal shockwave therapy (LiESWT) was first applied on chronic pelvic pain syndrome (CPPS) at 2009 in Europe and has just been used in Taiwan in these few years. We aimed to investigate the long-term outcomes of patients suffering from CPPS treated with LiESWT.
Materials and methods
This was an open-label, single-arm prospective study. 28 patients diagnosed as CPPS were enrolled. The patients were evaluated initially using NIH-CPSI score, visual analog scale (VAS), 5-item version of the International Index of Erectile Function (IIEF-5) and International Prostate Symptom Score (IPSS). LiESWT consisted 3000 pulses per session with frequency 4Hz were applied in each patient weekly for four weeks. The patients were re-evaluated immediately and 1, 3, 6, and 12 months after the LiESWT.
Results
Twenty of the 28 patients enrolled had completed the 1-year follow-up. Every aspect of NIH-CPSI score, VAS and IPSS significantly decreased immediately after the LiESWT. There were still significant differences in every aspect of NIH-CPSI score and VAS before and 1 year after the LiESWT while there was no difference in IPSS. There was no difference in IIEF-5 before and whenever after the LiESWT.
Conclusions
LiESWT is an effective therapy for patients with CPPS, which immediately benefits the patients in the pain, the urinary function and the quality of life. It also shows improvement in the NIH-CPSI score and VAS in long-term follow-up.
