Seminal vesicle Schwannoma presenting with nocturia

A 40-year-old man was referred to the hospital for evaluation of the left seminal vesical mass presenting with a 18-month history of nocturia and urinary frequency. He denied having constitutional symptoms including fever/chills, voiding difficulty or hematuria.
 
A soft and smooth mass in the region of left seminal vesicle separate from the prostate was palpated by digital rectal examination. Transrectal ultrasonography (TRUS) [Panel A] revealed a well-defined mass lesion, measuring 2.81 cm × 3.77 cm in size, homogeneous, hypoechoic echotexture in the region of left seminal vesicle. In this patient, computed tomography [Panel B] confirmed a well-demarcated solid mass at left seminal vesicle, size about 3.5 cm, with homogeneous contrast enhancement without adjacent organ invasion. Blood levels of electrolytes and results of renal- and liver-function tests were normal. Serum prostate specific antigen was 0.87 ng/ml and urine culture was sterile.
 
He underwent robotic assisted laparoscopic excision of left seminal vesical tumor. A yellow-white, elastic and encapsulated nodule [Panel C] was excised, and findings on histologic examination were consistent with Schwannoma. The patient was discharged on the fifth postoperative day and no impairment on erectile dysfunction was mentioned at follow up visits.
