病例報告:在陰莖腳罕見的陰莖骨折於一位曾動過睾丸切除術的病患

A rare penile fracture with a deep rupture over crus of penis in a patient with orchiectomy: A case report

Penile fracture, defined as rupture of the tunica albuginea of the corpora cavernosa, were mostly unilateral and found on the ventral aspect of the shaft because of the thinner tunica layers. Some special condition such as urethral injury, double penile fracture with one fracture on each corpora cavernosa, or double penile fracture near the root of penis was reported to be associated with vigorous sexual intercourse. Hereby, we presented a rare case of penile fracture with a deep rupture over left crus of penis in a patient with orchiectomy.

A 28-year-old case has underlying of left cryptorchidism and underwent orchiectomy during the teenager years. This time, he was presented to our urologic outpatient clinic with left scrotal hematoma for 3 days after the sexual intercourse. Physical examination showed severe swelling of left hemi-scrotum with tenderness. Sonography showed much hematoma over left hemi-scrotum. Further computed tomography (CT) scan showed a tear with hematoma at the left crus of corpora cavernosa. Operation was then arranged. During the operation, a deep rupture over left crus of penis was found which was compatible with CT scan. The morning erection was recovered within 3 days after the operation.

The only deep rupture over crus of corpora cavernosa is indeed rare. We suggested that we should be aware of the possibility of the deep rupture over crus especially in those whose testis is absent.
