The More Veins Ligated Intraoperatively; The More Pain Improved Postoperatively after Microsurgical Varicocelectomy

Purpose: Varicocele is a major cause of male infertility. However, few studies have discussed the connection between the pain cause by varicocele and pre-operative and intra-operative factors.
The aim of this study was to evaluate the factors possibly related to changes in pain score after microsurgical varicocelectomy.
Materials and methods: This retrospective study was conducted between September 2020 and October 2021 at China Medical University Hospital in Taichung, Taiwan. Patient characteristics including age, body mass index, semen analysis, testicular volume, and the number of varicose veins ligated were collected. Preoperative and intraoperative factors were analyzed to determine if they were correlated with changes in visual analog scale of pain (VAS) after microsurgical varicocelectomy.
Results: Thirty-four patients with clinical varicocele underwent subinguinal microsurgical varicocelectomy. Multivariate analysis revealed that younger age, smaller testicular volume, and the number of varicose veins ligated were significantly associated with changes in VAS after surgery. The area under the ROC curve of changes in VAS and the number of varicose veins ligated was 0.871. In addition, changes in progressive semen motility, non-progressive semen motility and sperm count after 3 months were significantly associated with changes in VAS.
Conclusions: The number of varicose veins ligated was associated with changes in VAS. In addition, semen motility and sperm count were also associated with changes in VAS.
