Unusual findings during inguinal hernia surgery: 2 rare cases

Purpose: To present our experience with unexpected findings during open or laparoscopic hernia surgeries, either unusual hernia contents or pathologic entities.
Materials and Methods: We study retrospectively over 400 patients with inguinal hernia who were admitted for open or laparoscopic hernioplasty over 8 years. In addition, our study included patients complaining of inguinal protrusion, even without a definitive diagnosis of inguinal hernia upon admission.
Results: Two patients presented with unusual hernia contents. One had a vermiform appendix in the sac during total extraperitoneal laparoscopic hernioplasty (Amyand’s hernia, type 1) (Figure 1). One diagnosed as pancreas adenocarcinoma with peritoneal carcinomatosis via pathology report of sliding hernia sac after open hernioplasty (Figure 2).
Conclusions: A hernia surgeon may encounter unexpected intraperitoneal findings. It is important to be prepared to detect then and apply the appropriate treatment.
