Retrospective analysis of anastomotic urethroplasty for pelvic fracture urethral distraction defect- 12 years’ experience

Purpose:
In order to evaluate the outcomes of perineal approach anastomotic urethroplasty; we provide a retrospective analysis of primary or redo posterior anastomotic urethroplasty for pelvic fracture urethral distraction defect (PFUDD) over 12 years at a medical center in Taiwan.
Materials and Methods:
The retrospective study was conducted in the Department of Urology, Linkou Chang Gung Memorial Hospital, Chang Gung Medical Foundation, Taiwan from January 2007 to October 2019. All these patients were undergone perineal anastomotic repair of PFUUD. We analyzed demographic findings, pelvic fracture type, distraction defect length (based on retrograde urethrography and cystography), micturating uroflow study, erectile function and urinary incontinence in postoperative phase.
Results:
A total 35 patients were included. Of these patients, 28 patients were primary cases and 7 (5%) were redo cases. The mean age of patients was 41.2 years and the mean distraction defect length was 4.2 center meter. Successful cases in primary urethroplasty were 24 (73%) and 6 (86%) in secondary urethroplasty case. Total success rate is 75%. The postoperative average peak uroflow rate was 15.75 milliliter per second (ml/sec) and the postoperative average residual volume was 36.7 milliliter (ml).
Conclusions:
In our study, our success rate of primary anastomotic urethroplasty is 76% and redo anastomotic urethroplasty is 85%. Total anastomotic urethroplasty success rate is 75%.
