En bloc transurethral resection versus conventional resection of bladder cancer

Purpose: We compare the detrusor muscle detection rates and complications between en bloc resection of bladder tumor (ERBT) and conventional TURBT (cTURBT).
Materials and Methods: A total 150 procedures performed by single surgeon between January 2016 and December 2019 at Kaohsiung Chang Gung Memorial Hospital were enrolled in this study, in which we tried to avoid the bias related to operator’s technique. Of these 150 procedures, 41 (27.3%) was ERBT and 109 (72.6%) was cTURBT. We retrospectively reviewed these medical records and performed statistical analysis.
Results: The detrusor muscle detection rate was significantly higher in the ERBT group, in which around 83% in ERBT and 43% in TURBT group (p<0.0001). In our multivariate analysis, the resection method is considered as an independent factor of detrusor muscle presentation (p<0.001).
Conclusion: ERBT provides higher detrusor muscle detection rate comparing with conventional TURBT. No severe complication was observed during our practice so far. ERBT emerges as a safe and feasible procedure for the diagnosis and treatment of bladder cancer.
