Abstract
Adrenal gland cysts are rare benign cystic lesions that incidence increasing with the extensive use of imaging technology. Without pathological examination, it is still difficult to differentiate an adrenal cyst from other adrenal neoplasms.
A 76-year-old man presented with a left asymptomatic adrenal cystic incidentaloma, and the tumor was non-functional. He was followed up by ultrasound for eight years and there was no apparent change of the cyst. The adrenal cyst ended up with progressive growth and malignant-like change after another seven years.
Our case showed that there is an opportunity for adrenal cysts to express malignant features. Surgical intervention remains proper management for definite diagnosis in an adrenal cystic neoplasm.
