Preliminary experience of erectile dysfunction treatment using penile platelet rich plasma injection.

Purpose
The aim of study was to report our preliminary experience of the treatment with platelet rich plasma in erectile dysfunction. We investigated the potential and safety of platelet rich plasma injection as a treatment for erectile dysfunction.
Materials and Methods
Between April 2018 to March 2020, the medical records of seventeen patients with erectile dysfunction, receiving penile PRP injection at Cardinal Tien hospital, were retrospectively reviewed, including patients underwent radical prostatectomy. Total 60 sessions of penile platelet rich plasme injection was performed. Effectiveness assessed according to the International Index of Erectile Fucntion 5 (IIEF-5), Erectile Hardness Score(EHS) at the baseline and after 3 sessions PRP treatment. We evaluated patient’s subjective satisfaction one week after each session.
Results
Total 17 patients under at least 3 sessions of penile PRP injection, including one patient with prostate cancer, receiving high-intensity focused ultrasound. The mean age of the patients was 59.58(SD=7.657) years old; mean testosterone were 326.23 ng/ml (SD = 124.84); mean PSA was 1.24ng/ml (SD=0.78) and mean time of erectile dysfunction was 27.88 months (SD=19.739). 70% of patients receiving oral pharmacotherapy, PDE5-ihibitor prior to penile PRP injection. There are 41% and 12% of patients among them having testosterone supplement and intracavernous PGE1 treatment, respectively. There are 3 patients receiving 6 sessions of treatment. After completed penile PRP injection treatment, a statistically significant difference between the before and after the treatment with regard to the quality of erection as measured by the IIEF-5 (p<0.05, 95%CI=(-6.18—-1.81) and the EHS (p=0.007, 95%CI=-0.3552—-4.243). No significant adverse event was reported.
Conclusions
The single arm, retrospective study showed preliminary experience of penile PRP significantly improves erectile function.
