Acute vasitis: a rare case misdiagnosed as lymphadenopathy

Introduction:
Vasitis is a rare condition that was usually related to chronic obstruction or urinary pathogens infection of vas deferens. Clinical symptoms mimic inguinal hernia or epididymo-orchitis and is often under diagnosed. We share our experience about a case of acute vasitis who was initially diagnosed as inguinal lymphadenopathy.
Case presentation:
A 32 years old male, with history of urolithiasis, initially presented with tenderness mass over left inguinal region. He experienced fever episodes up to 39 degree Celsius and lab data during emergency room visit showed leukocytosis but negative finding of urine analysis. Renal sonography and x-ray of kidney, ureter, and bladder showed no specific finding. Blood culture and urine culture did not reveal any pathogen. His symptoms slightly improved after 7 days of oral Ceftibuten used but inguinal mass persist. Therefore we arranged sonography and vasitis was confirmed. After switching to intravenous Ciprofloxacin, his symptoms and inguinal mass improved.
Conclusion:
There were many manifestation of vasitis, such as inguinal hernia or just presented with inguinal pain. We should always kept the diagnosis in mind while practicing in order to prevent non-necessary biopsy or operation.
