Hydronephrosis after renal transplantation: case report

A 57-year-old man had renal transplant 3 months ago. Decreased urine output and general fatigue was noted. Creatine rose to 8.94 mg/dL. Sonography showed hydronephrosis. Percutaneous nephrostomy and antegrsde pyelography was performed, but antegrade double-J insertion was failed due to guide wire cannot pass through ureterovesical junction with severe stricture (figure 1).
We performed cystoscopy and guide wire was inserted into ureterovesical junction difficultly (figure 2). Guide wire cannot advanced into the renal pelvis under fluoroscope. Therefore, we tried antegrade guide wire insertion and successfully advanced into bladder. Finally, Double-J was insertion in the position. We think this patient had problem of not only ureterovesical junction stricture but also ureter redundant cause hydronephrosis.
