Clinical profiles and laboratory variables of erectile dysfunction : variation between different age group

Objectives:
The increasing incidence of erectile dysfunction in men younger than 40 years has been reported recently. Etiology difference between younger and older ED group has been purposed currently, which mostly focused on psychogenic disparity. This study aimed to characterize the possible etiology of younger and older ED patients by comparing clinical profile and laboratory variables.
Materials and Methods:
The medical chart of 936 patients with ED (Erectile dysfunction) in NCKUH from 2016 to 2021 were retrospectively reviewed and analyzed. Subjects were categorized into two groups: age ≤40 years(210 patients) and>40 years (726 patients). The difference of clinical variables among two age groups was analyzed by the Student t-Test and Chi-Square Test. A P value less than 0.05 was considered significant.
Results:
Most ED patient was older than 40-year-old, accounts for up to 77.6% of our total ED patients. With penile doppler, the prevalence of vascular ED was higher in older group (62.5% Versus 26%). Regarding this aspect, the proportion of arterial insufficiency and venous -occlusive disease of vascular ED were also different in these two groups. Also, a significant difference in triglyceride level, HbA1c, renal function, prolactin, and testosterone levels was noticed between the two age groups. However, there was no difference in HASD-anxiety and depression scores between the different age groups.
Conclusions:
There is some discrepancy of clinical profiles and laboratory variables between the two age groups. Dissimilarity of penile doppler manifestation also support the suppose of different etiology of ED in these two groups. We demonstrated more hyperprolactinemia and penile arterial insufficiency in young ED compared to old ED.
