Urologic symptoms and burden in Male Ketamine Abusers: a large-scale questionnaire study

Purpose: To evaluate the prevalence of sexual dysfunction and lower urinary tract symptoms (LUTS) in the street and hospital illicit male ketamine abusers (KA).
Materials and Methods: The male street KAs caught by policemen and patients visited urologic clinics were invited to answer a structured questionnaire including international prostate symptoms score (IPSS), interstitial cystitis symptoms and quality index (ICSI and ICQI). Sexual dysfunction (SD) was defined as International index of erectile function (IIEF-5)≦21.
Results: Mean age of all 1318 enrollments (1211 street, 107 hospital KAs) was 27.4 ±6.2 years with male predominance. Higher scores of IPSS, ICSI and ICPI were observed in hospital than street KAs. (ICSI+ICPI ≧12) was more frequently observed in hospital than street KAs (77.6% vs. 6.7%, p<0.001). Mean IIEF-5 was significantly lower in hospital than street KAs (18.7±4.7 vs. 22.4±4.0, p<0.0001). Male SD was more frequently observed in hospital than street KAs (69.6% vs. 28.0%, p<0.01). Multi-variate analysis disclosed that risk factor for male SD were age ≧30 years (OR=1.765) and hospital KAs (OR=7.46). Female SD was reported more frequently, but not statistically significant, in female hospital than street KAs (85.2% vs. 66.7%), and no risk factors were identified. (ICSI+ICPI) was negatively correlated with IIEF-5 in male KA and weighted FSFI in female KA.
Conclusions: Male and female sexual dysfunction was frequently observed in the street and hospital ketamine abusers. LUTS was frequently observed in the hospital ketamine abusers, but less observed in the street ketamine abusers.
