A case report of complete degloving Injury of the penile skin caused by a pig bite

Abstract:
Injuries involving the degloving of male genitalia are uncommon, with animal bites being a rare cause. This case report details a rare instance of complete degloving injury to the penile skin following a pig bite. A 45-year-old male presented to our emergency department with severe trauma to his genitalia after a pig attack while assisting in collecting semen from the pig 4 hours ago. Upon examination, it was revealed that the whole layer skin of the penis had been avulsed except a 0.5cm attachment, exposing the underlying tunica albuginea. Immediate surgical intervention was undertaken to debridement and primary closure along with apical epidermal growth factor (NewEpi®) sprays on the soft tissues, and the patient was subsequently managed with antibiotics and wound care for 3 weeks. However, the gangrene and necrosis over the epidermis were observed, and as a result, he underwent a second operation to utilize split-thickness skin graft from the inner thigh, which was then sutured along the penile shaft. On the sixth day post-operation, the penile skin had completely healed, resulting in a satisfactory functional and aesthetic outcome. Though primary closure is possible in some cases, split-thickness skin graft may be considered the salvage therapy of choice.
