Case Report: Prostate cancer with penile metastasis

Case presentation:
A 91-year-old man with history of adenocarcinoma of prostate (Gleason Score:
5+5) suffered from urethral stricture and urine retention. He received radiotherapy over 10 years ago
and transurethral resection of the prostate three times in 2023.03, 2023.10 and 2023.12. At urology clinic, whole body bone scan revealed multiple bone metastases in. He came to our GU OPD follow up, and point induration at right side glans penis was palpated. Due to suspected prostate cancer
with penile metastasis, we consulted clinical radiological oncologist. We were suggested to get pathology prove for the penile hard nodule. Serial follow up of PSA level showed rising from 165 ng/ml to 479 ng/ml in recent three months. Penile biopsy under local anesthesia was done and the pathology revealed metastatic adenocarcinoma consistent with a metastatic adenocarcinoma from previously resected prostatic cancer. Therefore, he was referred to radiological oncologist for palliative radiotherapy.
Discussion:
Metastatic prostate cancer most commonly spreads to lymph nodes in other parts of the body or to the bones. It can also spread to other organs such as the lungs. Penile metastasis of malignant tumors is a very rare condition and is associated with poor clinical prognosis. According to previous studies, palliative radiation for the penile metastasis seemed to be a feasible therapy for local treatment. We herein report a 91- year-old male with prostate cancer and penile metastasis.
