Microdissection Testicular Sperm Extraction: initial experience of VGHTC

Purpose: Microdissection Testicular Sperm Extraction (micro-TESE) can be used for sperm retrieval (SR) or diagnosis. To our knowledge, there was no mTESE for azoospermia or severe oligospermia in mid-Taiwan. We try to set up this operation for such patients in our hospital.
Materials and Methods: Azoospermia has two group, obstructive type and non-obstructive type. For infertility patients, we checked hormone level, sperm analysis, and gene analysis. For those patients with normal hormone function level and azoospermia, micro-TESE will be suggested for diagnosis and sperm retrieval. If obstructive azoospermia was noted, vasovasostomy (VV) or vasoepididymostomy (VE) will be performed. For patient with severe oligospermia, micro-TESE is also suggested for sperm retrieval. mTESE, comparing with conventional TESE, dose not lose too much testis parenchyma.
Results: From October 2017 to March 2018, there were 11 patients underwent micro-TEST. There were 7 azoospermia and 4 oligospermia. Sperm retrieval revealed that we found sperm in 4 patients with oligospermia but 3 patients with azoospermia.
Conclusions: micro-TESE is a procedure of diagnosis and treatment for male infertility. We set up this operation in our hospital. We’ll follow up the result of fertilization and pregnancy rate in the future.
