Prostate cancer is quite common in male and was known as usually metastasize to bone. Cutaneous is a rare metastatic site for prostate cancer. Only a few cases have been reported in the past. We reported the case of 83 years old male who was diagnosed prostate cancer in 2009 and had received radiotherapy and androgen deprivation therapy in the local medical department. He first came to our clinic due to elevated PSA with mCRPC status in 2016. MRI was arranged and showed suspect multiple bone metastasis. After received 4 cycles of chemotherapy with Taxotere, we have shifted the treatment to Enzalutamide. However, he still felt uncomfortable and a new skin mass about 1cm*1cm black papule like lesion with progressed enlargement was noted over right thigh. Metastatic adenocarcinoma was diagnosed by skin biopsy. After diagnosed, patient had received radiotherapy at local medical department and under Enzalutamide. The skin mass was reduced without obvious enlarged after treatment. He was quite well during following up.
