Minimally Invasive Glans Augmentation Using Injectable Acellular Dermal Matrix Paste With or Without Hyaluronic Acid: A Preliminary Clinical Study

Purpose:
To evaluate the clinical efficacy and safety of glans augmentation using injectable acellular dermal matrix (ADM) paste through a minimally invasive approach, and to compare outcomes between Megafill (MF) alone and Megafill combined with hyaluronic acid (MF+HA).
Materials and Methods:
Between March 2025 and December 2025, 26 patients underwent glans augmentation under local or intravenous anesthesia. An 18G needle was inserted through the coronal sulcus, followed by sharp dissection of the subepithelial cavernous tissue using the needle tip. ADM paste (MF, with or without HA) was injected into the dissected plane, and the puncture wound was closed with 4-0 nylon. Patients were followed up for one month, and parameters including glans circumference and shape were recorded. Complications were also evaluated.
Results:
A total of 26 patients were analyzed (MF-only group, n = 15; MF+HA group, n = 11). The MF-only group (n=15) showed a mean immediate circumference increase of 1.36 ± 0.77 cm, while the MF+HA group (n=11) demonstrated 1.48 ± 0.41 cm. At one month, A total of 15 patients were analyzed (MF-only group, n = 7; MF+HA group, n = 8). Mean changes were 1.17 ± 0.45 cm and 1.19 ± 0.35 cm, respectively, indicating stable short-term volume with minimal reduction. No infections, extrusion, or sensory disturbances were observed, and transient edema resolved spontaneously. One patient reported ecchymosis. Eighty-eight percent of patients reported improved aesthetic proportion and satisfaction.
Conclusion:
Injectable ADM paste, with or without HA, for glans augmentation via an 18G needle provides a safe, simple, and effective method for achieving natural contour and measurable enlargement. Combining ADM with hyaluronic acid may enhance short-term volume maintenance, supporting its potential role in comprehensive penile aesthetic surgery.
