Antipsychotics induced ischemic priapism: Case report

Purpose: Ischemic priapism is a urologic emergency that rarely seen on the side effect of antipsychotic drug. We present a case report of antipsychotics induced ischemic priapism.
Case report: A 27-year-old male patient had medical history of insomnia and depression mood under regular antipsychotics for several months. He visited our emergency department with presented first episode of persist erection for night hours after taking antipsychotics. He had no history of sickle disease or any other hematological diseases. Physical examination at emergency department showed full rigidity of corpora cavernosa without ischemic change on appearance. Penile blood gas analysis showed dark blood, acidosis (pH: 6.97) with hypoxia (pO2: 17 mmHg) and hypercapnia (pCO2: 130 mmHg). Aspiration of penile blood with 18 French needle was performed. 50 ml dark blood was aspired at each side of corpora cavernosa. Penile pain relieved after the procedure and there was no erection afterward. He was discharged after eight hours of observation at emergency department. The diagnosis of antipsychotics induced ischemic priapism was confirmed by history taking, physical examination and penile blood gas evaluation.
Conclusion: In this case we remind the urologists that antipsychotics may induced ischemic priapism.
