Abstract
We present here a suspected case of traumatic high-flow priapism (HFP) in a 26-years old Taiwanese man, after scrotal injuries with the handlebar of stationary bicycle. After diagnosis by angiography, appearance of an active extravasation of contrast medium to cavernous spongiosum at regions of bulbous branch of right internal pudendal artery with identified fistula was reported. The condition was treated successfully through embolization by using Gelfoam. The post-embolization arteriogram revealed that fistula was no longer visible. Further, a penile detumescence and flaccidity was observed along with restoration of normal erectile function. Finally, it is advocated that embolization is a safer, efficacious and minimally invasive treatment of leaking pudendal artery to resolve such abnormal condition.
