Acute interstitial nephritis induced bilateral spontaneous urinoma

Abstract:
Urinoma a collection of urine encapsulated at the retroperitoneum, mostly associated secondary to trauma or ureter obstruction, but still can occur spontaneously.
We present a case of a 28 year-old woman with a past medical history of migraine, treated with over the counter analgesics. She complained of sudden onset epigastric pain and bilateral flank pain. Abdomen CT revealed bilateral urinoma without any obstruction. Foley insertion for decreased of bladder pressure and retrograde ureterscopy with Double J catheter insertion at bilateral ureter. Pain progressively improved.
Renal biopsy showed acute interstitial nephritis with nonsteroid anti-inflammatory drug (NSAID) associated. Urinoma is rare, while spontaneous urinoma is even less, our case showed that acute interstitial nephritis associated to NSAID can induce bilateral spontaneous urinoma.
