Objectives: Reporting a case with acquired concealed penis which was managed with dorsal and ventral slit as an ambulatory surgery.
Case presentation: A 65 year-old with uncontrolled diabetic mellitus visited our department with chronic balano-posthitis and buried penis for 3 years. Urination problem with hygiene issue bother him much recently. PE showed scarring contracture of the prepuce which caused a fissuring-like preputial ring. Surgical intervention with a simple longitudinal stricturotomy with transversal closure (Heineke-Mikulicz principle) on the doral and ventral side under local anethesia as an ambulatory surgery. The glans was exposed after the operation.
Result: Despite that the penile shaft was not fully exposed, the patient feels satisfied as the major concern was buried penis related difficult urination and hygiene issue.
Conclusion: Adult buried penis is an acquired condition related to obesity, circumcision, chronic inflammation, or lymphedema. Correction of the deformity improves quality of life. There were multiple techniques introduced to correct this situation, which were usually composed of complex surgical steps or even multi-staged surgery. Herein, we reported a case achieving patient-satisfied outcome using a relative simple procedure under local anesthesia.
