Inguinal bladder/ureter hernia with obstructive uropathy: A case report

Objectives: Reporting a case with inguinal bladder/ureter hernia related obstructive uropathy which the renal function regained after surgical intervention.
Case presentation: A 96-year-old male visited our ER for fallen down injury. The patient had past history of CHF, anemia, and BPH, s/p laser TURP in 2011. There were no head injury or cerebral vascular event after general check-ups. Nevertheless, an irreducible mass bulging at right inguinal area was incidentally found during Foley insertion. Lab data showed deteriorated renal function (Creatinine: 2.09 mg/dL; estimated GFR: 29.7 mL/min). The patient claimed that the mass lesion was found for years with unfixed size. Abdominal CT scan revealed herniated urinary bladder and right distal ureter into the right inguinal cannel, leading to right hydronephrosis and hydroureter. Surgical intervention with bladder hernia reduction and bassini repair was performed.
Result: Followed renal function got back to normal range after the surgery. The patient discharged uneventfully after the operation.
Conclusion: Bladder/ureteral hernia may cause obstructive uropathy which could be corrected with surgical reduction of the herniated contents. Image studies play an important role in the diagnosis of such situation.
