低能量震波(LISWT)對勃起功能障礙(ED)的治療效果能持續多久？一篇平均追蹤八個月的前瞻性研究

How long does the effect last of low intensity shock wave treatment (LISWT) for erectile dysfunction (ED)? A prospective study with an average 8 months follow-up

Purpose: LISWT was been introduced for ED treatment 10 years ago, and several studies have found LISWT may change the result for PDE5-I non-responders, including our study which had published on Urological Association of Asia Congress (UAA) last year. We keep extend the study, and discuss the long-term effect of LISWT with an average 8 months follow-up.
Materials and Methods: The patients were enrolled in our study between Mar. 2017 to Jul. 2018. All patients used PDE5-I but non-respondent (EHS≤2) or poor-respondent (EHS>2). Erectile function was assessed twice (pre-treatment and post-treatment) with IIEF-5 questionnaire and EHS scores. Our protocol was 6 treatments within 6 weeks, 4000 shock waves to the penile shaft and penile crus with energy flux density (EFD) 0.16 mJ/mm2 , and focus penetration depth set to 10 - 15 mm. The criteria for treatment success was defined as EHS above 2 points evaluated 1 month post-treatment.
Results: A total of 63 patients were enrolled in our study with median age 61 years. At 1 month post-treatment, EHS 3 points were 33 patients (52.4%) and EHS 4 points were 17 patients(19.1%). The immediate success rate was 79.3% (p < 0.001 ), and during the average 8 months follow-up (0-16), the cumulative success rate was 69%. No significant adverse events were observed during treatment and follow-up session.
Conclusions: LISWT may alter and improve the result for ED patients with previous poor response to PDE5I. And the therapeutic effect can sustain at least 8 months. Based on our results, LISWT could be an alternative treatment for ED.
