Squamous cell carcinoma of scrotum is a rare malignant scrotal tumor with an overall incidence which is reported to be less than 10 cases per year in United States. In scrotal malignant tumor, squamous cell carcinoma is the most common malignant tumor that reported 27% of scrotal tumor. It may associate with occupational exposure, HPV infection or poor hygiene. Herein, we presented a 96-year-old man with a huge scrotal tumor with multiple cauliflowers like lesion, which is resembled to condyloma acuminatum. The tumors enlarged gradually within six months. Scrotal tumor excision was performed. The tumor was measured 4.3 x 3.4 x 1.8 cm in size. The pathological report showed squamous cell carcinoma. Computed tomography was performed and found no lymphadenopathy in pelvis or around aortic region. Squamous cell carcinoma rarely arises from the scrotum. It may mimic condyloma acuminatum. The possibility should be kept in mind in those patients who presents with large cauliflower like lesion which may require wide excision and pathologic prove.
