Robot-assisted laparoscopic excision of retroperitoneal ganglioneuroma surrounding important blood vessels - case report and review of the literature

Ganglioneuromas are rare benign tumors of sympathetic nervous system, arising from the primordial neural crest cells that form the sympathetic nervous system. We present the use of the da Vinci Surgical System for the laparoscopic surgical treatment of a retroperitoneal ganglioneuroma that surrounding important blood vessels.
Case 1 is a 59-year-old woman was referred from LMD. One hypoechoic lesion that was near the inferior vena cava and superior to the right renal vessels was incidentally detected by ultrasound during a health check-up. Abdominal CT scan discovered a 9 cm right adrenal mass with calcification extent to retropancreatic region and surrounding IVC. pre-op CT guided biopsy was done and pathological report: suspected neurogenic tumour. Robot-assisted laparoscopic excision of the retroperitoneal tumor was performed on June 02, 2015 with total Operation time is 420 minutes and an estimated blood about 900 mL. Case 2 is 40-year-old man, who received health exam and MRI on 2016/06/27 showed a 12 cm retroperitineal mass. Pre-op CT showed a retroperitoneal mass with encase of right 3rd lumbar artery and anteriomedial displacement of IVC and biopsy showed neurogenic tumor. Robot-assisted laparoscopic excision of the retroperitoneal tumor was performed on Sep 13, 2016 with total Operation time is 390 minutes and an estimated blood about 200 mL. Both patients recovered well and followed up at our hospital.
Our case showed that huge retroperitoneal tumor encasing the major vessel can be excised using robot-assisted technology. An advanced robotic surgeon along with an experienced surgical team were require for these rare cases.
