Quality of life is greatly affected by pelvic organ prolapse, the prevalence of which is increasing because of the persistently growing older population. The tension-free vaginal mesh kit has grown in popularity owing to its comparable cure rate to traditional reconstructive surgery and the feasibility of an early return to normal life. The goal of surgery for pelvic organ prolapse is the restoration of anatomic defects.

The objective of this study was to report the anatomical outcomes of my initial experience comparing tension-free mesh-reinforced with sacrospinal ligament suspension and anterior colporrhaphy with mesh supporting in stage III or IV (Pelvic Organ Prolapse Quantification System/International Continence Society) uterovaginal prolapse and cytocele. The tension free mesh group includes 2 women aged 60-65 year-old with transvaginal cystocele and uterovaginal prolapse repaired using a tension-free mesh with sacrospinal ligament suspension, while the anterior colporrhaphy with mesh-supporting group includes another 2 women. The primary outcome was objective success < stage 1 prolapse. The secondary outcomes were postoperation pain score, subjective success rates, patient satisfaction with the surgery and complications. The overall objective success rates (in all compartments) were 100 % (2/2) in the tension free mesh group and 100 % (2/2) in the anterior colporrhaphy with mesh-supporting group. The development of a urinary tract infection, and new-onset stress urinary incontinence were not noted two groups. Buttock pain (temporary sciatic neuralgia) when sitting after operation 1 week was noted in one woman in tension free mesh group.
