Castleman Disease over Adrenal Fossa

Purpose:
Castleman disease is a rare lymphoproliferative disorder, usually in the chest or neck. Therefore, it is interesting if Castleman disease happened in adrenal gland. There were only a few case reports in literature, here we reported our case of Castleman disease over adrenal fossa and reviewed similar cases in our hospital.
Materials and Methods:
A 27 y/o female presented an irregular left adrenal tumor without abnormal adrenal gland function. Laparoscopic adrenalectomy since malignant adrenal tumor was favored from CT findings. The pathology report yield an unicentric Castleman disease besides the normal adrenal gland. We reviewed unicentric Castleman disease cases in our institution from 1990 to 2023.
Results:
There were totally 40 unicentric Castleman cases in our hospital from 1990 to 2023. Only 1 happened in adrenal gland. Most of the 40 cases were asymptomatic and were incidentally found from image surveys. Among the 40, 22 were male, and 18 female. The age of male case was from 12 to 83 (median: 60 y/o; mean: 51 y/o), and the age of female cases was from 9 to 71(median: 36 y/o; mean: 40 y/o). Castleman disease happened most commonly in the neck, followed by groin areas. A palpable mass was about 40% of our cases.
Unicentric Castleman disease usually is benign and surgical resection is enough for the disease.
Conclusion:
Unicentric Castlmen disease over adrenal fossa is rare and surgical resection is enough for disease control. Most of the cases in our institution were in neck and asymptomatic.
