Present Illness:
A 36-year-old man presented at the emergency department with progressive pain in his right lower abdomen that had persisted for two days. In addition, he experienced nausea, vomiting, and diarrhea. During the physical examination, the patient had right lower tenderness accompanied by rebounding pain.
Image and lab:
The laboratory results showed leukocytosis, while the electrolyte and renal function were within the normal range. An abdominal computed tomography scan was performed, which revealed an ectopic kidney with hydronephrosis in the lower abdomen.
Operation:
Although the left kidney was positioned correctly, the right kidney was absent, resulting in a diagnosis of ectopic right kidney with hydronephrosis. Since the ectopic kidney was non-functioning and causing abdominal pain symptoms, laparoscopic right nephrectomy was performed to remove the ectopic right kidney. The pathology report indicated kidney tissue with inflammation. After the surgery, the patient's right lower abdominal pain improved, and he recovered well.
Conclusion:
In conclusion, ectopic kidney is a unique anatomical variation where the kidney is positioned abnormally in the body. While this condition is generally asymptomatic and doesn't necessarily cause health issues, it's crucial for medical professionals to be aware of such variations for accurate diagnoses and treatment planning. Surgical indication includes: hydronephrosis, recurrent infection or stone impaction, pain, torsion or vascular anomalies.
