Case report: urethral cavernous hemangioma

Introduction:
Cavernous hemangiomas are unusual tumors that mainly distributed in skin and liver but rarely involving genitourinary system. We presented a rare case of urethral cavernous hemangiomas with symptoms of painless gross hematuria after ejaculation.
Presenting of case:
A 28-year-old male patient without past history, presented with painless gross hematuria after ejaculation for 5 years. Urine examination revealed neither hematuria nor pyuria. Sonography of urinary system was done and disclosed smooth urinary bladder wall and no evidence of hydronephrosis. Semen analysis showed milky white appearance with normal semen parameters. No hematospermia was found. Pelvic MRI was performed and revealed no visible cystic lesion in prostatic gland and adjacent structure.
A vascular mass located at posterior aspect of membranous urethra distal to the verumontanum was found by further diagnostic cystoscopy. Transurethral resection and electro cauterization were performed successfully. Microscopically, the vascular mass was composed of cystically dilated thin-walled vessels filled with blood and lined by flat endothelium, and highlighted by Factor VIII special stains. Cavernous hemangioma was confirmed. The patient returned to outpatient department smoothly without complaint about gross hematuria or hematospermia.
Discussion:
Urethral cavernous hemangioma is a rare and easily misdiagnosed disease. Less than 100 cases have been reported in the literature so far. The disease is mainly presented with gross hematuria, which is highly misdiagnosed because of the numerous diseases associated with this symptom. In female urethral cavernous hemangioma, it may cause symptoms as vaginal bleeding or urethral masses. A study indicated that urethral cavernous hemangioma can be treated by with local injection of pingyangmycin.
