Refractory ischemic priapism :A Case Report with Review of Literature

Abstract:
Priapism is a urological emergency defined as a prolonged,painful erection lasting greater than 4 hours.It occurs without any sexual stimulation.Several causes have been attributed to ischemic priapism, such as sickle cell disease, spinal cord injury, substance use and medications.We report a case of a 44-year-old man with history of depressive disorder with antipsychotics of trazodone and risperidone.This time, he had experienced about persistent and painful erection for more than 20 hours.Cavernosal injections and percutaneous T-shunt procedure were performed. Intially, the condition improved. The symptoms worsen after 6 hours of surgery.The emergent surgery arranged again for T shunting for ischemic priapism. We reviewed the literature of refractory priapism. From the case, we should be aware of possibility of refractory priapism and treatment.
