Case report: Extramammary Paget’s disease of the scrotum

A 79-year-old male presented to the urology clinic with progressive tenderness of right scrotal skin for six months. Physical examination showed erythematous crusted plaques with white scaly margin over right hemi-scrotum. Topical steroid and antibiotics were initially prescribed but in vain. Skin biopsy was performed and pathology revealed large, pale, vacuolated atypical cells in the epidermis, suggesting Paget’s disease. Abdominal CT showed no evidence of underlying malignancy nor lymph node metastasis. Right scrotal wall excision was performed with safe margin confirmed by intraoperative frozen section. Final pathology report confirmed extramammary Paget’s disease. The patient was under follow-up for five years by now without recurrence.
