Genuine overactive bladder is not common in children complaining isolated frequency

Purpose: Child with isolated frequency was less explored before. We used frequency volume chart (FVC) and uroflowmetry to investigate the prevalence rate of genuine overactive bladder.
Materials and methods: Patients complaining frequency without urgency, incontinence and other lower urinary tract symptoms were prospectively enrolled. Urinalysis was performed to rule out urinary tract infection. Then, they were requested to do 48-hour FVC and (uroflowmetry+ PVR) twice. Patients with congenital neurological and urological disorders were excluded.
Genuine-OAB is defined as maximal voided volume (MVV) less than 2/3 of expected bladder capacity (EBC) in FVC and bladder volume (voided volume +PVR) <2/3 EBC in uroflowmetry. Bladder over distention is defined as MVV >100% EBC. 
Results: In 9 years, 30 children were enrolled. Of them 3 did not record adequate FVC and were excluded. Mean age of the remaining 27 children (M: F=15:12) were 7.9 years. Based on FVC, 6 (22.2%) children had a MVV <2/3 EBC in both day and night and 4(14.8%) had a MVV <2/3 only in the daytime, but not the night time. In addition, the other 10 (37.0%) children had at least one voided volume > 100% EBC. Of 26 first uroflowmetry 20 (76.9%) was associated with a bladder volume <2/3 EBC. Of 22 second uroflowmetry 10 (50%) was associated with a bladder volume <2/3 EBC. Finally, genuine-OAB was observed in only 3 (11.1%) children. Of the 6 patients with MVV <2/3 EBC in day and night, one had repeat non-bell shaped curves at adequate voided volume. Repeat non-bell shaped curves were not observed in children with adequate MVV.  
Conclusions: Genuine-OAB and bladder over distention were observed in 11% and 37% of children complaining isolated frequency, respectively. Frequency volume chart should be used as first line screening tool and uroflowmetry is indicated in those with a MVV less than 2/3 of EBC in day and night.
