Glycemic control improved the effect of instillation therapy in patient of non-muscle invasive bladder cancer

Purpose: To evaluate the association between DM and glycemic control with prognosis of non-muscle invasive bladder cancer (NMIBC).
Materials and methods: We retrospectively analyzed 64 patients diagnosed clinical stage T1, high grade bladder cancer who received instillation therapy between 2007 and 2015. Demographic information including age, gender, ASA, diabetes (DM) or not, tumor characteristic, type of instillation therapy, and HbA1c level were recorded. Univariate Cox regression analysis was used to analyze the factors influencing survival. Kaplan-meier analysis was used to evaluate recurrence free survival, progression free survival and overall survival.
Results: 19 of 64 patients were diagnosed DM. The mean age was 67.1 year old, male accounts for 79.7% of all patients, and the mean follow up months was 39.9, There is no significant difference between DM, and non-DM group in demographic information except for proteinuria(p<0.0001). Cox univariate analysis showed DM significantly affects recurrence free survival and overall survival while CIS differentiation reaches significance in progression free survival. In 3-year recurrence free survival rate, DM poor control group (HbA1c>7) significantly inferior to non-DM group (55%,vs 87% p=0.01), while no difference between DM good control group(HbA1c<7) and non-DM group(p=.42). Compared to non-DM group, DM poor control group had significantly inferior survival rate (Log Rank test, p=0.008), while DM good control group did not reach significance difference (Log Rank test, p=0.41)
Conclusion: Glycemic control by reducing HbA1c level below 7 improved the recurrence free and overall survival in patient of NMIBC who underwent instillation therapy.
