Giant urinary bladder stone is a rare clinical presentation in the recent urological practice. Males are more affected than females. Bladder calculi are usually observed secondary to bladder outlet obstruction, neurogenic bladder dysfunction or foreign bodies. Here we report a case of 44-year-old male with history of mental retardation and neurogenic bladder status post suprapubic cystostomy admitted to our hospital with repeat urinary tract infection. Morganella morganii was confirmed in urine culture. Abdominal plain radiography showed a giant bladder stone. Computed tomography revealed a huge bladder stone. An open cystolithotomy was performed. A stone 10x9x7 cm in size was removed. Stone analysis showed carbonate apatite components.
