How to achieve 200+ cases durability with flexbile ureteoscopes: a singel hospital experience

Purpose:
To describe a novel operative method for extended durability and improved ease of use of current flexible ureteroscopes.
Material and methods:
328 patients underwent retrograde intrarenal surgery at our hospital. The modified method was used by a single surgeon. The outcome and durability for flexible ureteroscopes were assessed.
Results:
An Olympus digital flexible URF-V ureteroscope was used, and it required repair thrice during the study period. During factory maintenance, distal working channel damage was noted twice and outer bending rubber damage was noted once. Despite the damage and repairs, the deflection system was almost entirely intact after high-frequency use.
Conclusion:
The durability of a flexible ureteroscope (FURS) determines the efficacy of retrograde intrarenal surgery for renal stones. In this report, we described our modified upside-down technique for manipulation of the FURS in unequal dual deflection in order to preserve the deflection apparatus, which yielded greatly prolonged durability. Additionally, the use of mimic drive turning has decreased the time needed to train urologists.
