Introduction: We presented a case of eccrine poroma on the scrotum. Eccrine poroma originates from cells of the outer layer of the acrosyringium and terminal eccrine duct. It is commonly found on palms and sole or sides of the foot, occasionally on hairless surfaces of the extremities, chest, back, head, and neck. The scrotum is an extremely uncommon primary location.
Case presentation: A 68-year-old man had a tumor on the left side of the scrotum for a year. The tumor grew asymptomatically and there was no obvious pain or any other symptoms. Physical examination revealed a soft, pedunculated, and well-circumscribed tumor about 3 cm in length (Figure A). The pathology specimen under microscopic examination showed well-demarcated intraepidermal proliferation of poroid cells demonstrating monomorphous and cuboidal cytomorphology with oval nuclei and intercellular bridges (Figure B).
Discussion: Eccrine poroma is benign nature and it affect adults between fourth and sixth decades. Though rarely, it has the potential of malignant transformation which occurs at 18~50% of eccrine porocarcinoma. Eccrine porocarcinoma are seen more often in the elderly, with a female predominance.
